چکیده

EARLY ENTERAL NUTRITION IS THE standard of care in critically ill patients receiving invasive mechanical ventilation. However, numerous studies have shown that early enteral nutrition is frequently not used or associated with inadequate calorie delivery. The main reason for nonuse is gastrointestinal intolerance to enteral nutrition, which has been ascribed to gastroparesis with increased gastric volume, gastroesophageal reflux, and regurgitation or vomiting carrying a risk of aspiration and ventilator-associated pneumonia (VAP). This theoretical sequence has prompted a recommendation to monitor the residual gastric volume of mechanically ventilated patients receiving early enteral nutrition. When the residual gastric volume exceeds a predetermined cutoff, gastric prokinetic drugs are given and enteral nutrition is decreased or stopped to minimize the risk of aspiration and subsequent VAP. For editorial comment see p 283. Author Affiliations and a List of the CRICS Group appear at the end of the article. Importance Monitoring of residual gastric volume is recommended to prevent ventilator-associated pneumonia (VAP) in patients receiving early enteral nutrition. However, studies have challenged the reliability and effectiveness of this measure.

Nosocomial Infection in Critically Ill Patient Receiving Stress Ulcer Prophylactic Drugs

Acidic pH of stomach, which is a normal physiological barrier against bacterial overgrowth, would increase by stress ulcer prophylaxis initiation and may lead to bacterial colonization and play as a source for infection transmission to the respiratory system which results in ventilator related pneumonia in patients admitted to the Intensive Care Units (ICUs). Therefore, finding methods to ...

Comparing the Effects of Orthodentol and Chlorhexidine Mouthwash on Prevention of Ventilator-Associated Pneumonia in Patients of Intensive Care Unit: A Randomized Controlled Clinical Trial

Background and Aim: Ventilator-associated pneumonia is a common, serious and costly complication in patients of intensive care units. Oral hygiene is one of the important clinical measures to reduce the amount of pulmonary infection in these patients. Therefore, this study compares the effects of chlorhexidine and orthodontal mouthwashes on prevention of pulmonary infection in patients with end...

INTRODUCTION This systematic review and meta-analysis aimed to evaluate the effect of small bowel feeding compared with gastric feeding on the frequency of pneumonia and other patient-important outcomes in critically ill patients. METHODS We searched EMBASE, MEDLINE, clinicaltrials.gov and personal files from 1980 to Dec 2012, and conferences and proceedings from 1993 to Dec 2012 for randomiz...
